I/We wish to resign our membership at Colony Swim Club effective for the current club membership year.
If you wish to resign your Couple or Family membership, you must also send the completed form (listed above) to [email protected] by March 15 in order to receive your full membership certificate refund. Requests received after this date, but before April 15 will forfeit $50 of the certificate refund. Request received after April 15 will forfeit $100 of the certificate refund.

I/We wish to change our membership at Colony Swim Club effective for the current club membership year.

Please note that in changing your membership type you are acknowledging and agreeing to all of the following including the Guest Policy as it pertains to members.

By selecting below you agree to all of the following.

1. Once a family membership is relinquished it will be offered to a new family on the CSC waiting list and may not be reclaimed without applying for and re-joining the waiting list.
2. Both named members on the membership agree to the change in membership and will remit a signed form attesting to that fact.
3. In the event that there were previously children under age 25, living within the household of the member, upon converting to a Couple or Single membership, I understand that our choice to downgrade our membership from Family to Couple or Single is not reversible, and acknowledge membership changes including but not limited to the following:
4. Children are not eligible to join Colony swim team
5. Children are not eligible for "Blue Dot" and guests must be accompanied by the member at the pool at all times.
6. Children are not eligible for swim lessons
7. Grandparents are not allowed on the membership
8. Guest fees will apply for any guest at the Club other than myself (Single) and my spouse (Couple).
